There are forms to be completed for both burial of cremated remains and for the introduction of a plaque, or an added inscription to an existing plaque.  There are also fees to be paid in accordance with the Church of England Fee Table, which is accessible on the Church of England website.  A  fee for grave digging is charged by the Parochial Church Council.
