If you save it and open in a pdf reader such as Adobe Acrobat, the form can be completed onscreen, saved and emailed back with a scanned drawing of your planned memorial. Otherwise, it can be printed, completed by hand and posted back for approval.

The application is subject to St Mary's Churchyard Regulations, also downloadable from this page.

Please return your application either by :

emailing the completed form to admin@astburychurch.org.uk

or by posting to Memorial Applications, St Mary's Church, Peel Lane, Astbury, Congleton CW12 4RQ

Following approval of your application by the Rector, we will send you an invoice for the fee and upon receipt of your payment, permission will be granted to proceed with erection of or addition to the memorial. Please contact us to arrange a mutually convenient time for this work in the churchyard to avoid coinciding with any other churchyard event.
